About CareSentinel
CareSentinel is an independent service that compiles the public CMS Care Compare record for U.S. nursing homes — the federal survey, deficiency, penalty, and ownership record, as published by the Centers for Medicare & Medicaid Services — and presents it without alteration or judgment.
What CareSentinel is
A faithful copy of the public federal record, organized so a family can actually read it. Every facility page restates what CMS has published — the inspection citations, the penalties, the ownership file — dated to the exact federal data release it came from. CareSentinel computes no score of its own: there is no CareSentinel rating, ranking formula, or risk model anywhere on this site, and that is by design (see our methodology). What CMS published is what you see.
Where the data comes from
Every figure on this site is copied from the nursing-home datasets the Centers for Medicare & Medicaid Services publishes in its Provider Data Catalog. The snapshot in use is the 2026-08-01 edition. Six files are used: Provider Information, Health Deficiencies, Penalties, Ownership, State and US Averages, and the Citation Code Look-up.
Every figure traces to one of those files, loads as a single dated snapshot, and carries a visible snapshot date on the page. The full account — CMS’s own definitions for every term we display, how snapshots are versioned, and how to reproduce any figure yourself on medicare.gov — lives at /methodology.
How often it is refreshed
CMS republishes these files monthly, and this site is rebuilt from the newest snapshot after each release. Every page carries the snapshot date it was built from, so a page and the CMS file behind it always agree. Between refreshes nothing on the site changes.
Which facilities get their own page
For each facility we count how many record lines occur at that facility and nowhere else in the country. A facility with six or more such lines gets its own page. A facility below that threshold still gets its own page if it carries any federal fine, any Medicare payment denial, any deficiency cited at scope-and-severity G or above, or a Special Focus Facility designation. Every remaining facility is carried as a single row on its county page, anchored so that a link to that facility lands on its row.
These pages are generated automatically
Facility, county and state pages are assembled by a program directly from the CMS files named above. No one writes prose for an individual facility. Numbers, dates, tag codes and deficiency wording are reproduced as they appear in the source files; they are not rounded, reworded or interpreted. Where a value is missing from the source, the section that would have carried it is not rendered at all.
Legal-advertising compliance
CareSentinel is a legal-advertising service structured for ABA Model Rule 5.4. It is not a law firm and does not provide legal advice. It does not evaluate, recommend, or vouch for any attorney’s qualifications; participating attorneys independently evaluate every prospective matter and keep full professional independence. CareSentinel’s compensation is a flat advertising fee — never a share of case revenue, never a contingency split, never a referral fee.
Who operates this, and the standard we hold
CareSentinel is built and operated by SJ Lee through Nodal Logics LLC, a Wyoming company. It is a small, independent operation — not a marketing network, not a call center, and not a venture-backed directory. One person is accountable for what appears on these pages, and that person can be reached directly at records@caregiverhelpnow.com. The operation is run from the Republic of Korea and has no office or staff in the United States.

This is a commercial mail-receiving address used to accept United States postal mail. It is not an office.
Everything on a facility page is transcribed from one source: the provider data files that the Centers for Medicare & Medicaid Services publishes for every certified nursing home in the United States. We download a CMS snapshot, load it whole, and render it. We do not survey facilities, we do not accept payment from them, we do not take submissions from them, and we do not compute a rating or a ranking of our own on top of the federal figures. Each page names the snapshot month it was compiled from, so any number on it can be checked against the same public file.
The standard we hold ourselves to is fidelity: we add no characterization of our own to the federal record — no spin toward alarm, and no softening either. When we fall short of that standard — if a page misstates what the CMS file for its snapshot month actually says — we want to know. The correction protocol on /methodology says exactly how corrections are handled, and /privacy and /terms set out how inquiries and personal information are treated.
If you’re here about a loved one
Plainly: CareSentinel is an independent service that lets you read a nursing home’s public federal record and, if you choose, put an inquiry on file to be connected with a participating attorney. If you’re matched with a participating attorney, CareSentinel is paid an advertising fee. We are independent of CMS, Medicare, HHS, and every other government agency; we are not a nursing home, and we are not a government complaint line. Everything you read here is the public CMS record — the same record anyone can verify on medicare.gov.

Independence & affiliations
CareSentinel is independent of every government agency whose data it compiles, and of every facility it lists. Figures come from public federal data; the federal record can understate what actually occurred, and inspection findings are point-in-time survey results — not a determination that any specific resident was harmed. The full disclosures appear in the footer of every page.
