What we collect

If you submit an inquiry, we collect what the form asks for: your name, phone number, the state and dates involved, the type of harm, your relationship to the resident, and the facility your inquiry concerns. We never ask for your Social Security number, date of birth, home address, or medical records.

How we use it

Your inquiry is used to connect you with a participating attorney in our network. As qualified attorneys join our network in your area, your information may be shared with one participating law firm so it can contact you about your inquiry. We do not sell your information, and we do not share it with more than one firm at a time.

Facility data

The facility records on this site are compiled from public federal data published by the Centers for Medicare & Medicaid Services (CMS). They are public records, not personal information.

Retention and your choices

We keep inquiry information only as long as needed to route it and meet legal obligations. You may ask us to delete your inquiry at any time, and we will honor that request except where retention is legally required.
