The federal record

Facility pages restate public federal survey, deficiency, penalty, and ownership records as published by the Centers for Medicare & Medicaid Services (CMS). The data is provided as is; CareSentinel does not warrant the accuracy or completeness of governmental data, and the federal record may understate what actually occurred. CareSentinel draws no conclusion of its own about any facility.

Inquiries

Submitting an inquiry does not obligate you to retain any attorney, and does not guarantee that an attorney will contact you. You remain free to consult or retain any attorney of your choosing. Strict time limits apply to these claims — a qualified attorney can evaluate the deadlines that apply to your situation.
